This is a new, open box - it was installed and then removed. When they removed it, they scratched the end piece that connects to the wall. However, it's mostly hidden with the escutcheon piece. Other than that, Isa is in great condition! Please see all photos. Everything in the photos is included. It comes with the base plate, but not the screws—any wood screw will work, though.
