There are four levels total with the three buttons working for each individual area. There is one noticeable imperfection (hole) on the outside of the box on the back (pictured). There are two noticeable imperfections on the larger cardboard cutouts, one being on the Bob-omb battlefield/cool, cool mountain and the wet-dry world/tall-tall Mountain cutout in the middle on the reverse side (see photos), as well as the middle insert piece on the Bob-omb battlefield/wet-dry world cutout section on the reverse side.
